    Step 3.

    Before a student can be enrolled at South Summit High School, the following information MUST be provided:

    1. Proof of residency (for example a utility bill, etc. that shows current name and address), which proves you live within school boundaries.
    1. Proof of guardianship (if necessary). If living with anyone other than a biological parent, then transfer of guardianship documents MUST be provided. In case of a divorce, court documents MUST be provided showing that the parent has legal custody of the student.

    1. Immunization records
    1. Transcript/s from past school/s attended REQUIRED for scheduling classes. (High School)
